Original Contractor Requirements/Checklist - Home Improvement Commission

After you pass the MHIC exam, we need the following supporting documents needed to process your Contractor application:

• Complete the Co-owner signature page and include your signatures and date at the top and bottom of the form: https://www.labor.maryland.gov/forms/mhiccoownsig.doc
• Complete the Personal Financial Worksheet: https://www.labor.maryland.gov/forms/mhicpersfinstatement.doc
• Certificate of Insurance. The Certificate Holders box must read:

  MHIC
  100S Charles St., Tower 1
  Baltimore, MD 21201

• Three months recent personal bank statements
• Full individual credit report
• Trade name registration including Articles of Incorporation: https://egov.maryland.gov/BusinessExpress/EntitySearch
• Sign your surety bond at the BY:            if you are not financially solvent by $30,000. The Bond template may be found at this link: https://www.labor.maryland.gov/forms/mhicsuretybondform.doc
• Personal photo. A selfie or a clear copy of your driver's license will do.
